  • AMA UK – WAQFE NAU ('NEW DEDICATION') ADULT’S PERSONAL DATA CONSENT FORM FOR ADULTS (13 YEARS OR OVER)

  • ‘WHAT IS THIS CONSENT FORM FOR?’

    As a member of the Ahmadiyya Muslim Association UK (‘AMA UK’), your consent is required to process and share some of your personal and sensitive (‘special category’) data for the purposes listed below. Under data protection law, AMA UK is a Data Controller. This form enables you to give your consent to AMA UK to process and share your personal data.

    Please note, this is different to the consent that you may have given to AMA UK via other forms, such as your Tajneed (Membership) Form, Marital Assistance (Rishta Nata) form, etc. This form concerns the ‘Purposes’ for processing set out in the Table on page 2 of this form.

    ‘WHICH TYPES OF PERSONAL DATA WILL YOU PROCESS & SHARE?’

    By signing this form, and ticking the boxes in the Table on page 2 below, you will provide AMA UK with your consent to process and share the following categories of personal data:

    Identity Data

    Data relating to your identity, for example your first name, last name, marital status, title, date of birth, gender and membership (AIMS) number.

    Contact Data

    Data relating to how we contact you, for example your billing address, correspondence address and email address.

    Religious Data

    Data concerning your religious beliefs and practices. For Ahmadi Muslims, this may include details of: when you took the initiation (Bai’at); when you signed the membership (Tajneed) form; and (if applicable) when and/or how you stopped being an Ahmadi Muslim. This is also part of your special category data.

    Transactions/ Donations Data

    This includes details of payments you make in relation to your membership of AMA UK, including charitable donations. It may also include, in some cases, details of products you have purchased from us.

    Special Category Data (i.e. Sensitive Data)

    The EU General Data Protection Regulation 2018 defines special category data as “data revealing your racial or ethnic origin, political opinions, religious or philosophical beliefs, or trade union membership […] genetic data, biometric data for the purpose of uniquely identifying a natural person, data concerning health or data concerning your sex life or sexual orientation.”

    Educational Data

    This includes Identity Data, Contact Data and Special Category Data. Further, (if applicable) this includes data relating to: requests for funding for studies; details of ongoing studies; educational performance; student loan status, as well as details concerning awards, certificates, qualifications or degrees awarded

    Waqfe Nau Data

    This may include Identity Data, Contact Data, and Education data in relation to children. This may also include details concerning education and training preferences.

    ‘HOW WILL YOU COLLECT MY PERSONAL DATA?’

    We collect your personal data via the following means:

    1. From you via your direct interactions with AMA UK (e.g. when you fill in forms or correspond with us);
    2. Through AMA UK membership administration processes (e.g. via the activities of AMA UK Departments and Office-bearers); and
    3. From third parties (e.g. the Joint Data Controllers mentioned below) or publicly available sources.

    ‘HOW WILL YOU USE MY PERSONAL DATA AND WHY?’

    We will process your personal data for the purposes set out in the Table on Page 2 below. Full details as to how and why we use your data are contained in our Privacy Notice at www.ahmadiyyauk.org/gdpr. You can request a hard-copy of our Privacy Notice by contacting us via our contact details (see below on Page 2).

    ‘WHO WILL PROCESS MY PERSONAL DATA?’

    In order to fulfil the Purposes mentioned below (see Table on Page 2), AMA UK needs to process and share some of your personal data with the following Joint Data Controllers:

    • Ahmadiyya Muslim Jamaat International (‘AMJ International’) (London, UK);
    • Tahrik Jadid Anjuman Ahmadiyya (Rabwah, Pakistan);
    • Sadr Anjuman Ahmadiyya (Rabwah, Pakistan);
    • Waqf-e-Jadid Anjuman Ahmadiyya (Rabwah, Pakistan);
    • Sadr Anjuman Ahmadiyya (Qadian, India).

    We ensure all appropriate safeguards are in place in order to transfer personal data to Joint Controllers outside of the European Economic Area (‘EEA’) irrespective of UK leaving the EU and not following GDPR . In particular, AMJ International and the other Joint Controllers have adopted the standard data protection clauses published by the European Commission (see Article 46(2)(c) GDPR).     ‘WHAT ARE MY DATA PROTECTION RIGHTS?’

    You have the right to withdraw your consent at any time without detriment. This shall not affect the lawfulness of processing based on consent before its withdrawal. Please note that all processing of your personal data will cease once you have withdrawn consent, other than where this is required or permitted by law. Your rights are fully listed in our Privacy Notice (see Page 1 above).

    ‘WHO IS RESPONSIBLE FOR MY DATA AND WHO CAN I CONTACT?’

    • You may only fill in this form if you are 13 years of age or older. If you are under 13, your parent or guardian must fill in a Children’s consent form on your behalf, which is entitled: ‘AMA UK – WAQFE NAU CHILDREN’S PERSONAL DATA CONSENT FORM’.
    • Separate consent forms must be completed for each AMA UK member. e. One consent form per member.

  • AMA UK – WAQFE NAU ('NEW DEDICATION') CHILDREN’S PERSONAL DATA CONSENT FORM FOR CHILDREN (UNDER 13 YEARS)

  • ‘WHAT IS THIS CONSENT FORM FOR?’

    As your CHILD is a member of the Ahmadiyya Muslim Association UK (‘AMA UK’), your consent is required to process and share some of your child’s personal and sensitive (‘Special Category’) data for the purposes listed below. Under data protection law, AMA UK is a Data Controller. This form enables you to give your consent to AMA UK to process and share some of your child’s personal data.

    Please note, this is different to the consent that you may have given on behalf of your child to AMA UK via other forms, such as their Tajneed (Membership) Form, Wasaya ('The Will' ) form etc. This form concerns the ‘Purposes’ for processing set out in the Table on page 2 of this form.

    ‘WHICH TYPES OF PERSONAL DATA WILL YOU PROCESS & SHARE?’

    By signing this form, and ticking the boxes in the Table on page 2 below, you will provide AMA UK with consent to process and share the following categories of personal data in relation to your child:

    - If your child is 13 years old (or older) they must complete their own consent form for adults. This adult consent form is entitled: ‘AMA UK – WAQFE NAU ADULT’S PERSONAL DATA CONSENT FORM’.
    - If your child is under 13 years old, you must fill in this form on their behalf. Your child cannot fill in this form themselves.
    - Separate consent forms must be completed for each child. i.e. One consent form per child.
